docs: remove outdated troubleshooting workarounds

Removed v3.0.x migration guide, old idleTimeoutSeconds workaround
(auto-handled by setup.mjs since v3.2.1), and manual session cleanup
instructions. Replaced with concise troubleshooting for common issues.
